Electronic hookah vaping reduces plasma concentrations of nitrate/nitrite and increases systemic oxidative stress

Abstract Background Electronic (e-) hookah–a new category of vaping devices–has increased in use among young adults recent years. E-hookahs utilize the placement e-bowls on traditional water pipes, allowing flavored aerosol to pass through a water-filled base before being inhaled. Though advertised as safe tobacco alternative, e-hookahs deliver nicotine by creating an fine particles and other free radicals that may increase cardiovascular disease risk. While acute e-hookah impairs endothelial function, underlying mechanism is not completely understood. Purpose To determine mechanistic involvement nitric oxide (NO) oxidative stress induced dysfunction. Methods Healthy adult chronic hookah smokers (n=7; 26±1 years age, 24.4±0.8 kg-m2; mean ±SE) were randomized vape two 30-minute sessions sham vaping, separated 7-day washout period. Flow-mediated dilation (FMD) brachial artery was performed with ultrasound. Circulating plasma nitrite/nitrate (NOx), index concentration, lipid peroxidation marker 8-isoprostane concentrations, well smoking exposure biomarkers (plasma exhaled carbon monoxide) measured after sessions. Results As compared unchanged parameters (p=ns), reduction FMD −27±3% (pre- vs. post-vaping, p<0.001), suggesting impairment function. These vascular changes accompanied significant reductions circulating levels NOx increases 8-iso prostaglandin F2a (−41±11% +28±11%, respectively; p<0.001). Plasma concentrations 4-fold more e-hookah, whereas CO did change (p=ns). Conclusions Our data demonstrate transiently systemic decreases levels, latter implies reduced NO bioavailability line observed expressed reductions.
